Documents Required

You may first download and print the Application Form. Attach self-attested photocopies of following documents:

 

·         Indicate which service is required by ticking on one or more of the services enumerated on the first page.

·         Passport in original with self-attested photocopy of its first two and last two pages.

·         Enclose a letter explaining the requirement of specific service by the Embassy.

·         Completely fill the form from column N°1 to 12 of Section-I.

·         Completely fill Section-II and sign in the space indicated (if applicable)

·         Please do not fill answers like NA, N/A, Nil or dash, kindly explain if you do not fill any column.

·         Enclose a copy of residence permit / DNI / cédula / visa (in case of DNI / Cedula Cards copies of front back sides should be enclosed)

·         2 Photos size 2” x 2”. One pasted in the designated space on the first page.
